  The consent is available for the nasal vaccine or the injectable vaccine. However, the injectable vaccine will only be offered in clinic. The consent forms are also available in other languages, ie Polish, Punjabi, Romanian, Ukranian and Urdu, available using the link above.

  Please complete the consent form as soon as possible to ensure that your child receives their protection this winter.
